Dear Consumer,
It is our highest priority to provide great quality products to our customers.
This notice is to provide details regarding the recall of Model/Sku Number #
490/30 and 21502/36+1 of our Chandelier products.
It has come to our attention that these chandeliers can fall from the ceiling
unexpectedly due to an inferior, manufactured “loop” at the top of the fixture.
The dates the problematic items have been sold were:
490/30: 1/1/07 through 6/25/10
21502/36+1: 9/26/08 through 8/12/09
The defective part in question is called the “loop”. See below photographs of
the defective loop. We need you to confirm if this is the loop you have.
(**Please note- this only applies to the loop in the enclosed photograph** Any
other loop is 100% ok and are not part of this recall**)
INSTRUCTIONS:
Please follow the following steps if your item does match the picture:
1) Please inform us with your order number to confirm that you *do* have the
loop as pictured.

2) Take a picture of the loop you have to verify you indeed have the incorrect
part.
3) Once we confirm you indeed have the incorrect loop you should IMMEDIATELY
turn the power off at the fuse box, and remove the chandelier.
